Description

The primary role of the Culture Care Connect After Care worker is but not limited to:

  • Provide aftercare services to Aboriginal and Torres Strait Islander individuals who have been referred following a suicide attempt/suicidal crisis in line with agreed Service delivery model.
  • Provide advocacy services that support and encourage clients to access a range of social health and welfare services, which include other Nunkuwarrin Yunti programs and external agencies.
  • Develop and maintain an extensive range of social support networks to assist with accessible and equitable pathways for Aboriginal and Torres Strait Islander families seeking appropriate services.
  • Provide inclusive, client driven case management (short and longer term) for people with complex and/or multiple social health needs.
  • Ensure strong linkages and brokerage activities are undertaken with internal and external services related to key performance outcomes of the program.
  • Work in partnership with counsellors, psychologists, social workers and clients within a social and emotional well-being casework framework.
  • Promotion of the service through networking, education and presentations to ensure the continued visibility of the service and ongoing referrals from other agencies.
  • Additional periodic or ongoing activities that may arise during the tenure of the role to meet client needs or otherwise as requested of the team manager.


Skills And Experiences

ESSENTIAL – includes qualifications, skills, experience and knowledge

  • A Minimum of Certificate IV recognised and relevant qualification (i.e. Community Services, Social & Emotional Well Being, Social Work, Mental Health or similar).
  • Comprehensive knowledge and understanding of social and emotional well-being issues affecting Aboriginal & Torres Strait Islander people in an urban setting.     
  • Building the capacity of local Aboriginal and Torres Strait Islander communities and service providers to care for and respond to social disadvantage, including suicides and traumatic incidents in their community
  • Experience in working with Aboriginal people and/or communities.
  • Experience in working in a primary health care service, community setting or human services organisation.
  • Experience in providing effective and well-informed advice, assessment, advocacy, liaison and referral to community services.
  • Proven well developed written and verbal communication skills and the ability to communicate effectively with clients, colleagues and external agencies.
  • Ability to liaise, network and negotiate with a range of human service organisations and government agencies.  
  • Ability to deal with people with high and complex needs and the ability to deal with conflict constructively.
  • Demonstrated experience in managing a diverse range of tasks and competing priorities, working with minimal supervision and working as a member of a team.
  • Competent in the use of information and communication technology including MS Office.

DESIRABLE

  • HLT40113- Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Cars or equivalent training and/or experience
  • Mental Health qualifications and/or experience
  • An understanding of the principles of Social Justice and Community Development.
  • An understanding of culturally sensitive practices.  
  • An understanding of the legal implications of client work.
